BASSI DATA CONTROLLER APPLICATION FORM
1. General Information
Under the Law on the Protection of Personal Data No. 6698 ("KVKK"), individuals defined as data subjects ("Applicant") are granted the right to make certain requests regarding the processing of their personal data in accordance with Article 11 of the KVKK.
The application form has been prepared to determine your relationship with BASSI DESIGN MOBILYA MIMARLIK KIMYA INS. IC VE DIS TIC. SAN. LTD. STI. ("BASSI") and to identify your personal data processed by BASSI, if any, by Masko, in order to respond accurately and within the legal deadline to your relevant application. Additional information may be requested by Masko for the purpose of identifying the identity and authority for identity verification and authorization. If the information provided by the Applicant is incorrect and/or not up-to-date, or if the requests are unauthorized, the responsibility lies with the Applicant.
In accordance with Article 7 of the Regulation on the Procedures and Principles of Applications to the Data Controller, no fee shall be charged for written responses to the Applicant's application up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page over ten pages. If the response to the application is provided on a recording medium such as a CD or flash drive, a fee may be charged equal to the cost of the recording medium.
2. Scope of the Right of Application Under Article 11 of the KVKK
The Applicant may apply to Masko and make requests regarding the following issues:
1) Learning whether personal data is being processed,
2) Requesting information if personal data has been processed,
3) Learning the purpose of processing personal data and whether they are used in accordance with their purpose,
4) Learning the third parties to whom personal data is transferred, whether domestically or abroad,
5) Requesting correction of personal data if it is incomplete or incorrectly processed and, within this scope, notifying third parties to whom personal data has been transferred,
6) Requesting the deletion, destruction, or anonymization of personal data if the reasons requiring their processing no longer exist, even though they have been processed in accordance with the KVKK and other relevant laws, and, within this scope, notifying third parties to whom personal data has been transferred,
7) Objecting to the occurrence of a result against oneself by means of the exclusive analysis of processed data through automated systems,
8) Requesting compensation for damages if personal data is processed unlawfully.
3. Application Method
In accordance with the first paragraph of Article 13 of the KVKK, applications made regarding these rights must be submitted to us in writing and signed or by other methods determined by the Personal Data Protection Board ("Board").
In this context, written applications will be made by printing out this form;
By the Applicant's personal application in person with identity verification document,
Through a notary,
Via registered electronic mail (KEP) address, secure electronic signature, mobile signature, or
By e-mail sent by the Applicant from the electronic mail address previously notified by the Applicant and registered in our system.

BASSI may request additional information to verify the identity of the Applicant in applications made via e-mail and may take necessary precautions.
In accordance with the second paragraph of Article 13 of the KVKK, applications submitted to us will be responded to in writing or electronically within thirty days from the date of receipt, depending on the nature of the request.
